This presentation examines why Serious Injuries and Fatalities (SIFs) persist despite improving safety records — and how Safety Resilience Engineering offers a practical, evidence-based framework to address them.
Traditional Safety-I approaches fail to capture the complex system interactions that drive SIFs. The session introduces Safety-II thinking and its four resilience pillars: uncovering deeper causal pathways through improved investigation methods; strengthening frontline adaptive capacity using human performance tools and crew resource management; redesigning reporting systems to detect weak SIF precursor signals; and applying cognitive risk mapping to identify invisible high-risk tasks conventional assessments miss.
